REGISTERING YOUR CHILD

Registration begins in February for the following school year.

Registration Requirements

• Birth Certificate/Passport (will temporary accept hospital record/document)

• Photocopy of parent/guardian ID

• Health Records - Form 14 - current TB, Varicella clearance, required immunizations, current school physical (need doctor note with appointment date/time if not completed by school start date)

• Proof of Residence - copy of utility bill, rental agreement, notarized letter (Proof of Residence Verification)

We welcome Geographic Exceptions (GEs) for all grades. GE request forms first need to be submitted to your home school (beginning Jan 1). Your home school will then process and forward your paperwork to PCHES.
